Waiver & Release of all Claims

I recognize and acknowledge that my child/ward will be participating in SkyART programs. I agree to indemnify, hold harmless, waive and relinquish all claims, including negligence, against SkyART, its officers, agents, employees and volunteers, including claims of bodily injury, possible death, and property damage as a result of, arising out of, or caused by, my child’s/ward’s presence and participation in the activities of SkyART.*

I acknowledge that SkyART does not provide health and accident insurance coverage for my child/ward. I agree that it is my responsibility for payment of any medical bills arising from injuries sustained, or any medical attention given, while my child/ward participates in the activities of SkyART.

I further acknowledge that any artwork my child/ward produces at SkyART remains the property of SkyART and hereby give my consent for the display and publication of any such artwork.

I hereby grant permission to SkyART to photograph, videotape, or otherwise depict the image of, my child/ward and consent to the display and publication of the same by SkyART, or its agent or appointee, for any purpose, including educational and advertisement purposes, and in any medium, including print and electronic. I understand that SkyART may use such photographs with or without associating names thereto. I further waive any claim for compensation of any kind for SkyART’s use or publication of photographs, videotapes, or other depictions of my child/ward.

In the event of a SkyART Field Trip, I grant permission for my child/ward to travel with a SkyART employee in the SkyART van or on public transportation to different cultural institutions in Chicago. I also give permission to SkyART to pick up my child/ward from school in order to participate in SkyART’s afterschool programs. I understand that I am responsible for picking up my child from SkyART after programs are finished. I am aware that SkyART does not provide health and accident coverage for any participant and it is my responsibility for any medical bills from injuries sustained while in transit.

In addition, I give permission for my child to walk to the SkyART Artist Garden, located on Brandon and 89th Street, with a SkyART employee in order to use the garden for art activities, harvesting food for cooking, and general gardening.
